Complete the online new patient form and choose Men’s Hormones. The clinic will contact you about your consultation, bloodwork, eligibility and next steps.

TRT Hero Rate & Referral Initiative
*Qualifying new patients receive Testosterone Replacement Therapy services at $125/month for the initial six months with a required six-month commitment. After six months, the rate transitions to $150/month. Treatment remains subject to medical evaluation and provider approval. Existing clinic patients receive a one-time $25 credit toward their next bill for each active first responder who signs up for any clinic service and provides the referrer’s full name during intake. There is no cap on qualifying referrals; credits may stack across future billing cycles. Credits have no cash value, are non-transferable and cannot be redeemed for cash.
